Research Interests:
Myopic Retinoschisis
Myopic retinal disorders including myopic choroidal neovascularization and myopic schisis are still not treated completely. Their management is still controversial and need further investigations. Dr. Baba is interested in myopic schisis and has reported its prevalence and characteristic in highly myopic eyes. [AJO 2002] Currently, the most common approach to myopic schisis is pars plana vitrectomy with ILM peeling. However, this procedure sometimes causes postoperative development of full-thickness macular hole which is hard to be treated even with repeated vitrectomy. Moreover, the retinal damage by ILM peeling has to be taken into account. We have reported the efficacy of scleral buckling surgery using macular plombe to treat myopic schisis. [AJO2005] Recently we have tried scleral imbrication combined with vitrectomy. The functional and anatomical outcome was favorable although ILM peeling was not performed.[Retina2016]

Morphological changes after macular surgery
The morphological change after ILM peeling during vitrectomy has been reported as dissociated optic nerve fiber layer (DONFL) appearance. Recently, this observation was called inner retinal dimpling and suggested the damage of inner retina. We have described the thinning of ganglion cell complex after ILM removal during vitrectomy for macular hole and it was associated with decreased retinal sensitivity. [Eye 2012] This morphological and functional change after ILM removal was not affected the types of dye (ICG, BBG) and is thought to be thoroughly due to mechanical damage. [Ophthalmology 2013] Although the ILM peeling contributed to the increase of success rate after vitrectomy for MH, the necessity of ILM peeling should be considered again. The retinal sensitivity and vascular density after ILM removal was determined by OCT and microperimetry. [Retina 2020] The reduction of vascular density at the temporal macula was correlated with low retinal sensitivity.

Kaoru Fujinami, MD PhD, is a clinical ophthalmologist and a Clinical Geneticist, working for the Laboratory of Visual Physiology, and Department of Ophthalmology/Medical Genetics Center, Tokyo Medical Centre (TMC). He is also affiliated with Genetics, University College London (UCL) Institute of Ophthalmology associated with Moorfields Eye Hospital.

He has focused on inherited retinal disorder (IRD) over the last 15 years. He has been working on improving understanding of IRD both in clinical and molecular genetic aspects, including clinical characterization, electrophysiologic assessment, molecular genetic analysis, genotype-phenotype association/correlation, and introducing cutting-edge treatments.

TMC leads many nationwide and international genetic research consortia, including the Japan Eye Genetics Consortium (JEGC), National Hospital Organization Network Research Group (NHO-NRG), and East Asia Inherited Retinal Disease Society (EAIRDS). These consortia have large cohorts in Japan/East Asia, and analyses of the obtained big data (N>3000 Japanese subjects, N>6000 East Asian subjects) yield novel findings, including ethnicity-specific phenotype and genotype. In 2020, the TMC team started a gene augmentation trial targeting RPE65-associated retinopathy, the first in East Asia. In addition, therapeutic options with advanced technology have been developed under intra- and international industry-government-academia collaborations.

Kyoko Ohno-Matsui
Tokyo Medical and Dental University
Kyoko Ohno-Matsui is Professor and Chair of the Department of Ophthalmology and Visual Science at Tokyo Medical and Dental University (TMDU). She is also the Chief of the Advanced Clinical Center for myopia at TMDU. She graduated from Yokohama City University Medical School and received her Ph.D. at Tokyo Medical and Dental University. She did postdoctoral fellowship at Wilmer Eye Institute at Johns Hopkins University.
Her research mainly focuses myopia, especially pathologic myopia. The High Myopia Clinic at TMDU was established in 1974, and now registered patients have exceeded 6,000 from all over Japan and from abroad. Based on the long-term progression study, she established international photographic classification of myopic maculopathy. She is especially interested in how eye is deformed in pathologic myopia, and then how eye deformity damages retina and optic nerve. As a method to visualize the shape of the entire globe, she established a new technique called ‘3D MRI of the eye’. Based on 3D MRI and wide-field fundus imaging, she proposed a novel classification of posterior staphyloma. Using various imaging modalities (especially swept-source OCT), she has been detecting structural abnormalities which occur in various tissues in eyes with pathologic myopia. Currently she is actively working on ultra-wide field OCT imaging to visualize the entire extent of staphyloma in many patients in vivo. Her lab also investigates basic mechanism of myopization and ocular angiogenesis.
Dr. Ohno-Matsui has been the recipient of Young Investigator Award in Japanese Ophthalmological Society and the Excellence in Research Award at TMDU. She has authored or co-authored 300 manuscripts which were published in prestigious journals. She is the active member of Macular Society, Retina Society, and Club Jules Gonin.
